Advance Care Planning Support: Advance Care Planning is a process of reflection and communication. It is a time for you to reflect on your values and wishes, and to let people know what kind of health and personal care you would want in the future if you were unable to speak for yourself. Advance Care Planning Workbook: This online interactive workbook has the necessary information about advance care planning to guide you through the steps of creating your individualized plan. Download it and share it with others. 

Alberta Health Services (AHS): Offers programs and services related to health care, including information on accessing medical equipment in Alberta (AHS Continuing Care Services).

Alberta Aids to Daily Living (AADL): Provides financial assistance to Albertans with a long-term disability, chronic illness, or terminal illness for medical equipment and supplies (AADL).

Edmonton Southside Primary Care Network: Passionate about creating healthier communities. Offers programs and classes for free or low-cost (PCN).

Health Canada: Offers general information on medical devices, health products, and safety guidelines (Health Canada - Medical Devices).

Medically Assistance in Dying Support: Medically Assistance in Dying (MAiD) is a legal option in Canada for adults who meet specific criteria set out in law. Is that right for you? Canadian Virtual Hospice is a great resource to help you with your feelings, questions, and information about how MAiD works, and its emotional impacts.

Seniors Health Benefits: Alberta Seniors Benefit, dental and optical assistance, and additional health benefits for seniors (Seniors Health Benefits).

Veterans Affairs Canada: Canada’s Veterans Affairs website provides information about services and benefits available to Canadian veterans, including case management, disability benefits, health care programs and benefits, mental health services, rehabilitation programs, etc. Veterans Affairs Canada also runs a 24-hour crisis helpline to help support veterans with “almost every problem that can affect your personal or professional life.” (VAC)

Canadian Virtual Hospice: The Canadian Virtual Hospice provides support and personalized information about advanced illness, palliative care, loss, and grief, to people living with illness, family members, people working in healthcare, educators, and researchers (Canadian Virtual Hospice).

Caregivers Alberta: Caregivers Alberta strives to empower caregivers and promote their well-being by providing resources, mental health support, and education for people caring for family members or friends. You can speak to a caregiver coach who will talk to you one-on-one, be a listening ear, support you, and help you navigate the system. This association also provides workshops to help caregivers balance their own needs with the demands of their caregiving role. (Caregivers Alberta).

City of Edmonton Program Services for Seniors: Edmonton helps seniors to be involved in the community and provides access to senior centers, support groups, and many other programs and services (Edmonton.ca). 

Edmonton Seniors Coordinating Council: Offers information on services, programs, and resources for seniors in Edmonton, including health and wellness support (Edmonton Seniors Coordinating Council).

Family Caregiver Centre- Alberta Health Services: The Family Caregiver Center is Calgary-operated and connects caregivers with accurate and up-to-date information, health care, and community services. The center offers information, education, and support to family caregivers.

Family Caregiver Wellness Workshops: Become a confident caregiver! NorQuest College offers a wide range of workshops taught by experienced healthcare professionals to help caregivers gain the confidence and knowledge they need to care for a loved one at home and/or needing end-of-life care.

Family Support: Family is Medicine. Ronald McDonald House Charities gives families a place to stay together, close to their sick child in hospital. Because when families stay together, sick children get stronger. 

SAGE Seniors Association: Provides programs, services, and resources to enhance the quality of life for seniors in Edmonton (SAGE Edmonton).

AHS- Together4Health: Your recovery and rehabilitation (regaining your strength) after COVID-19 can be challenging, but resources are available to help manage your symptoms and get help when needed (Long Covid- AHS).

Alberta Diabetes Foundation: The Alberta Diabetes Foundation funds innovative research to prevent and treat all forms of diabetes. We are aligned with talented researchers across Alberta, and progress is being made. Funding is disbursed when and where needed most, moving projects along that may otherwise be stalled, and facilitating measurable results (Alberta Diabetes Foundation).

The Arthritis Society – Alberta & Northwest Territories: Offers information on managing arthritis, local programs, and support services (Arthritis Society Alberta).

Bladder Cancer Canada: As the first and only Canadian patient advocacy organization dedicated to bladder cancer issues, Bladder Cancer Canada is proud to provide patients, their caregivers, and families with the support and resources they need to help navigate a bladder cancer diagnosis. – (Bladder Cancer Canada).

Heart & Stroke Foundation of Canada: Offers information about heart disease, stroke, and healthy living (Heart & Stroke Foundation).

Osteoporosis Canada: Osteoporosis is a disease characterized by low bone mass and deterioration of bone tissue, which can lead to an increased risk of fracture. Known as the “silent thief”, bone deterioration can occur over many years without presenting any symptoms. (Osteoporosis Canada).

Equipment Loan and Assistance Programs

Publicly Funded Care: Albertans can access publicly funded services by working with an AHS Case Manager; private for-purchase services are arranged and paid for by the individual. It is also possible to arrange a combination of public and private services to ensure all of your needs are met. AHS

Canadian Red Cross – Health Equipment Loan Program (HELP): Offers short-term loans of medical equipment such as wheelchairs, walkers, and bathing aids (Red Cross HELP - Alberta).

ALS Society of Alberta – Equipment Loan Program: Provides equipment to individuals living with ALS in Alberta (ALS Alberta Equipment Loan).

Seniors Safety & Housing

Alberta Seniors and Housing: Offers information on seniors' services and programs, including support for aging in place (Alberta Seniors and Housing).

Edmonton Seniors Centre: Offers programs and activities for seniors, along with information on health and wellness (Edmonton Seniors Centre).

​Edmonton Seniors Safe Housing: Free, safe housing for up to 60 days. Suites are furnished and include meals and necessities. Day staff provide information, support, and assistance as needed, including finances, transportation, housing, life skills, etc. Health assessment and basic health needs are provided by VON (Victorian Order of Nurses). Criteria: Seniors or senior couples, age 60 or over who need to leave an abusive situation. Must be able to live independently. 780-702-1520

Elderly Adult Resource Service (EARS): EARS provides support, assessment, and referral to elderly adults who are at risk of experiencing abuse or neglect. Criteria: This team's focus is supporting seniors who are experiencing low to medium-level risk in their home situation and where police intervention is not required. 780-477-2929

End-Of-Life Care Support: The End Of Life Doula Association of Canada has a vision to raise the standard of End-Of-Life Care. End-of-life doulas empower, educate, and encourage people and their families to be involved in making decisions. They see death as a process that can span over months or even years rather than an event and doulas can help with supporting and planning for the future, regardless of diagnosis or illness. Having a conversation or making a plan when you are healthy, is encouraged by doulas so that the care received at the end of life is appropriate and aligned with your wishes.

Family Violence Prevention Center: Assists with reducing barriers and provides individuals and families with a safe and supportive environment where they can make informed choices. Criteria: Services are available for culturally diverse backgrounds. 780-423-1635

Seniors Abuse Help Line: A phone line answered 24 hours daily by trained volunteers at the Support Network,​ who provide confidential and anonymous support, information, and referrals on elder abuse matters.​Criteria: Seniors experiencing abuse or anyone concerned about a senior who may be experiencing abuse. 780-454-8888​

Senior Care Support: Nav-CARE is a FREE program designed to help people living with an illness that isolates and overwhelms them. Specially trained volunteers, with connections to professional healthcare providers, work with seniors to access resources and services in their community while providing companionship and emotional support. Like a helpful neighbour or friend, Nav-CARE volunteers visit seniors in their home regularly with a focus on enhancing their quality of life. Nav-CARE supports seniors to live well and independently at home for as long as possible.
